Your Impact:
- Develop personalized care plans tailored to each member’s needs, ensuring the least restrictive treatment level.
- Partner with members, families, and providers to create effective care strategies.
- Provide individual and group therapy for adolescents and adults.
- Connect members with essential social services, including financial, psychosocial, and community resources.
- Advocate for members to ensure their voices and preferences guide treatment decisions.
- Apply clinical criteria to determine appropriate care levels.
- Maintain thorough documentation of assessments, care plans, and referrals.
- Meet performance goals and KPIs.
- Master the StrongWell model and drive outcome-focused care.
- Engage in interdisciplinary meetings and utilization reviews to support seamless care transitions.
- Uphold ethical stewardship of company resources.
- Demonstrate reliability through consistent attendance and punctuality.

What We Require:
- Active Louisiana licensure (LCSW, LMFT, or LPC).
- Substance abuse experience preferred.
- Background in maternal/substance abuse treatment a plus.
- Experience working with adults and adolescents.
- Knowledge of Medicare/Medicaid processes.
- Proficiency in remote work.
